- Complete the Transfer your super form (use one per super account)
- Make certified* copies of your proof of identity documents as listed below (one original set per super account)
- Send it to AustralianSuper, GPO Box 1901, Melbourne VIC 3001 and we’ll do the rest, including contacting the other funds.

Find your lost super
It's easy to lose track of your old super accounts, especially if you have changed jobs, your name or address.
Before you complete a Transfer your super form, you'll need to find the details of these lost super accounts.
There are a few different ways to help you find your lost super:
- Using your Tax File Number (TFN), the Australian Taxation Office SuperSeeker website can help find lost super free of charge. Alternatively you can call them on 13 28 65
- AUSfund also publishes information about unclaimed super. You can use their free online search service unclaimedsuper.com.au or call AUSfund on 1300 361 798; or
- Call the funds you think you may have an account with.
In general, having your old member account number and TFN will greatly assist you when searching for your lost super.

Completing proof of identity
You will need to provide documentation with transfer requests to prove you are the person to whom the superannuation entitlements belong.
The following documents include:

EITHER one of the following documents:
|
|
|
- A driver’s licence issued under State or Territory law; or
- A Passport.
|
|
|
OR a combination of the following documents:
|
|
|
- A birth certificate or birth extract
- A Citizenship certificate issued by the Commonwealth
- A Pension card issued by Centrelink that entitles you to financial benefits.
AND:
- A letter from Centrelink regarding a Government assistance payment
- A notice issued by Commonwealth, State or Territory Government or local council within the past twelve months that contains your name and residential address. For example:
- A Taxation Office Notice of Assessment
- A Rates notice from local council.

Have you changed your name or are you signing on behalf of another person?
If you have changed your name or are signing on behalf of the applicant, you will need to provide a certified linking document. A linking document is a document that proves a relationship exists between two (or more) names. The following table contains information about suitable linking documents.

Certification of personal documents
All copied pages of original proof of identification documents (including any linking documents) need to be certified as true copies by any individual approved to do so (see below).
The person who is authorised to certify documents must sight the original and the copy and make sure both documents are identical, then make sure all pages have been certified as true copies by writing or stamping ‘certified true copy’ followed by their signature, printed name, qualification (e.g. Justice of the Peace, Australia Post employee, etc) and date. The following can certify copies of the originals as true and correct copies:
- A permanent employee of Australia Post with five or more years of continuous service
- A finance company officer with five or more years of continuous service (with one or more finance companies)
- An officer with, or authorised representative of, a holder of an Australian Financial Services Licence (AFSL), having five or more years continuous service with one or more licensees
- A notary public officer
- A police officer
- A registrar or deputy registrar of a court
- A Justice of the Peace
- A person enrolled on the roll of a State or Territory Supreme Court or the High Court of Australia, as a legal practitioner
- An Australian consular officer or an Australian diplomatic officer
- A judge of a court
- A magistrate, or;
- A Chief Executive Officer of a Commonwealth court.

^AustralianSuper's administration fee will increase to $1.50 per week as of 1 January 2009.
*Important information: In calculating historical returns for AustralianSuper, where only one of the merging funds (ARF or STA) previously offered an equivalent to an option in the new product suite, the returns for that option have been used. Where both funds offered the equivalent option, the returns have been asset weighted across both options on a monthly basis, where possible.
