Complete this form by 20 February 2026 if you choose to keep your basic age-based Income Protection.
If we don’t receive this form by 20 February 2026 (and if you haven’t made any changes to your Income Protection or haven't notified us of a claim), we'll reduce your Income Protection to $1,000 a month of fixed cover.
This reduction will be effective from 28 February 2026. We'll write to you if this happens.
