What is a Superannuation standard choice form?
The Superannuation standard choice form is an Australian Taxation Office (ATO) form that allows you to nominate the super fund you'd like your employer contributions paid into.
You can use it whether you're starting a new job or updating your super fund details with your current employer. The form helps your employer capture the information they need to pay your super to the fund you've chosen.

When you might need it
If you’re starting a new job, your employer must pay your super into your stapled fund. A stapled super fund is an existing super account that is linked, or 'stapled', to you – it follows you when you change jobs. If you don’t have one and you don't nominate a fund, your employer can pay into a new account in their default super fund.
Alternatively, you can use the Superannuation standard choice form to tell your employer which superannuation fund you want superannuation contributions paid to. You may need a Superannuation standard choice form if:
- you're starting a new job and want your super paid into AustralianSuper
- you already have an AustralianSuper account and want future employer contributions paid into it
- you've joined AustralianSuper and need to tell your employer the super account details to pay into.

You can also choose your super fund online through your myGov account in ATO online services under Employment.
What you’ll need to complete the form
Before completing the form, you’ll make sure you have:
- your AustralianSuper member number
- your personal details and employment information
- AustralianSuper's fund details:
- Fund name: AustralianSuper
- USI number: STA0100AU
- ABN: 65 714 394 898

- Phone: 1300 300 273
If you're not yet a member and wish to choose to have your employer contributions made to AustralianSuper, you can join AustralianSuper first to get a member number. Applying should take less than 7 minutes.

How to complete the form
1. Download the form
Download the Superannuation standard choice form (PDF)
2. Complete your details
Fill in your personal information and nominate AustralianSuper as your chosen super fund.
3. Give the form to your employer
Give the completed form to your employer along with the Letter of compliance so they can update their payroll records and make sure super contributions go to your AustralianSuper account.
